To schedule your complimentary health exam, please call your nearest VCA Animal Hospital or visit their website to book your appointment online* within 14 days of your pet’s adoption date. We look forward to seeing you and your newly adopted pet soon.

Complimentary 14-Day Follow Up Care*
for any of the following conditions ($500 maximum coverage):

• Upper respiratory infections
• Suture removal
• Diarrhea of infectious origin
• Kennel cough
• Internal parasites
• Vomiting of infectious origin
• Ear and eye infections
• Common skin disorders
• Urinary Tract Infections

To activate Healthy Start Certificate
Present the following at time of appointment:

• A copy of this letter
• Copies of your pet’s adoption paperwork and medical records provided by the shelter.

Not included: Emergency and specialty veterinary services, vaccines, congenital and inherited diseases or conditions. Any diagnosis requiring additional treatment including Canine Parvovirus, FIP, Feline Leukemia, FIV, Distemper, Hepatitis and Ringworm, is also excluded.

*The medical services identified above provided by VCA Animal Hospitals are covered up to $500 during the first 14 days after adoption. This certificate is void should you select another veterinary hospital for the initial examination of your adopted pet. This offer applies to qualifying adopted dogs or cats only. Redeemable at a participating VCA Animal Hospitals only (see reverse for list of locations).
